If they won by 3, you’d lose. If North Carolina is then a +4 and they lost by three to Duke, you’d win because they were “given” 4 points according to the bet.

Totals are easy because it’s the combined score from the two teams in the game, set as an over/under wager. The total could be set at 148 and you could choose either Duke/UNC to combine for over that score, or under.
